Following documents are
required to be submitted for issue of a PIO Card:

Application form:
-
Duly completed in
DUPLICATE.
-
Application form should be
filled in fully, legibly (preferably in BLOCK CAPITAL LETTERS or
typewritten) and accurately.

The columns 13 and 14 MUST
be fully and carefully completed.
-
Applicant may mention the
details of parents, grand-parents, spouse etc., to the extent
possible bringing out their Indian or foreign origin as applicable.
-
For children, who cannot
sign, a thumb impression may be put in place of signatures in the
application form.
-
Existing valid passport,
in original.
-
A photocopy of the
existing valid passport.
-
Expired Indian
passport/travel document(s) of the applicant or either of his/her
parents or grand-parents or great grand-parents, in original, along with
photocopies.

All other relevant
documents and their photocopies that may be helpful in establishing
Indian origin which may include, inter alia, revenue records, domicile
certificate, school or college leaving certificates, foreign
naturalization certificate, Indian birth certificate of the applicant
and of the parents or grand-parents of the applicant, etc., nationality
certificates of the applicant and of the parents or grand-parents.

In case application is for
a child, Indian passports of parents (along with photocopies) and
notarized copy of birth certificate of the child may be submitted.

Eligible applicant, who is
applying as spouse of a person of Indian origin:

Should submit original
Indian passport or PIO Card (whichever is applicable) of the spouse.

Notarized copy of marriage
certificate. To be eligible, the marriage must have taken place more
than a year before the date of application.

Four recent passport size
photographs of the applicant showing frontal view of full face.
-
Notarized copy of proof of
residence is required.
-
Fee of US $380 (Bs.F
1,900) for adult applicant.
-
Fee of US $190 (Bs.F 950)
for a minor applicant below 18 years of age.
-
In case application is for
a PIO Card in lieu of a lost PIO Card:
-
Submit sufficient proof of
residence in the area of jurisdiction of this Embassy.
-
Above requirements are
indicative and attempt has been made to cover majority of cases.
However, the Consular Officer may ask for additional documents after
examining the application in person and the same would have to be
provided.
-
When sending by mail, only
notarized copies of the documents that are required to be submitted with
the application may be enclosed. No original documents should be sent
unless specifically required by the Embassy.

Applicant may include
return courier (with complete name, address, telephone numbers, etc), if
delivery of documents is requested by mail.
TO ENDORSE NEW PASSPORT NUMBER ON PIO CARD
-
A PIO Card shall be valid
for a period of fifteen years from the date of issue, subject to the
validity of holder's passport, which is endorsed on the PIO Card.
-
The PIO Card shall be
valid only if its holder presents it with his/her valid passport that is
endorsed on the PIO Card.
-
To endorse a new passport
number, both passport and PIO Card in original are required.
-
There is NO FEE for this service.
- There is no form for
this. Applicant may submit a request letter with his/her passport and
PIO Card.
